In the VascularVita longevity programme, healthspan is followed first through walking, daily function and independence. Stem-cell and exosome products are assessed on the basis of batch characterisation and regulatory status. Functional follow-up and a product claim are not interchangeable. The phrase regenerative approach is used widely enough to cover stem-cell applications, platelet-rich plasma, extracellular vesicles (EV) and exosome preparations, and experimental reprogramming studies.

How the stem-cell pool declines with age is described in stem cell exhaustion, and evidence tiers in evidence tiers. The file and storage side of an exosome batch is treated in evaluating an exosome technical dossier and cold chain and storage.

Three layers: basic care, indication, research

A regenerative discussion cannot be reduced to a single treatment package. In practice three layers coexist:

  • Basic function and risk management: Resistance exercise, adequate protein intake, sleep, control of metabolic risk, and guideline-based care of chronic wounds and vascular disease. This layer is the spine of a longevity programme.
  • Indicated clinical use: In a diagnosed disease, an authorised product or a treatment included in a guideline. Stem-cell transplantation in haematological disease, and cellular treatments in selected cartilage injury, belong here. The decision depends on disease stage, the centre’s competence and the regulation.
  • The research layer: Preclinical mechanism studies, early-phase clinical studies and biomarker panels. Data in this layer do not show that healthspan in the general population has been extended.

When the layers are mixed, a research exosome preparation is presented as if it were an indicated treatment. The layer to which a product belongs defines the limits of what can be claimed.

Regulation: which class is the product in?

In the European Union, cell- and tissue-based treatments are classified, under defined conditions, as an advanced therapy medicinal product (ATMP) and assessed under Regulation 1394/2007. The regulatory class of an exosome or EV product depends on the source, the manufacturing method and the intended use. In most countries, clinical use is either under a marketing authorisation or under an approved clinical-trial protocol. In Türkiye, classification and authorisation of the relevant products rest with the Turkish Medicines and Medical Devices Agency (TİTCK). Before clinical use, the regulatory status is confirmed in writing.

Evidence tier: where does the claim stop?

An authorised indication and clinical-outcome data (H1–H2) answer which procedure is in question for which patient. Observational biomarker data and small series (H3) generate a hypothesis. Animal and mechanism data (H4) set the direction of research.

Particle-size distribution, the surface-marker profile and protein content of an exosome batch are that batch’s characterisation data. They show that the batch was produced in a defined and consistent way; they do not prove clinical outcomes such as wound closure or walking distance. Mechanisms such as immune modulation and angiogenesis are discussed in the literature. Those mechanisms do not predict the clinical result of a given batch.

Exosomes and EVs: what characterisation shows

The terms EV and exosome should be interpreted together with the source cell, the isolation method and the analytical panel. Characterisation covers these headings:

  • Characterisation: Particle count, size distribution (nanoparticle tracking analysis, NTA, or an equivalent), a marker panel and batch-release criteria.
  • MISEV reporting: The MISEV guideline of the International Society for Extracellular Vesicles (ISEV) defines the minimum information to report in EV studies. The MISEV2023 update clarified measurement and naming rules. Comparison between batches is possible because of that reporting.

Product scope is listed on the stem cell and exosome page. In vascular disease the revascularisation decision follows the order in the medical and endovascular pathway in peripheral artery disease; regenerative products do not replace that decision.

Points often confused in clinical practice

Functional screening and disease treatment. ICOPE and frailty assessment show loss of function. The result of that assessment is not an indication for an exosome application.

Promotional language and the IFU. Wording in a product brochure does not extend the indication in the instructions for use. Brochure text is no substitute for an authorised indication.

The chronic wound. A chronic wound is assessed first for perfusion and infection (perfusion assessment, antimicrobial dressings). In a wound whose perfusion problem is unsolved, no biological product produces healing on its own.

Summary

Regenerative approaches are considered in three layers: basic function and risk management, indicated clinical use, and research. Batch characterisation shows the quality of the product; a clinical study shows effect. One is not used in place of the other.
